Important reminders:
Your DOT physical should cost $100 or less. Any remaining funds are for your permit test, so call around to find the most affordable provider.
A drug test is NOT required for your DOT physical. A urine sample will be taken for your overall health. Requesting a drug test will unnecessarily increase the cost.
Important: On your Medical Examiner’s Certificate, check “YES” in the CLP/CDL Applicant or Holder section (bottom right corner). FreeWorld cannot approve DOT physicals without this box checked.
What you need to bring to your physical
- Driver’s License
- Glasses or contacts, if applicable
- All medications in their prescribed packaging
- Your DOT/DMV funds
Getting to your appointment
We encourage you to explore all local transportation options — public transit, carpooling with family or friends, or walking — before using a rideshare service.
If no other options are available, you can request an Uber through the FreeWorld app. Please note that Uber should be used as a last resort due to limited availability.
